How do I create the Evaluation Note Templates?
Purpose:
To guide therapists in creating standardized evaluation note templates for consistent clinical documentation in HelloNote EMR.
NOTE: To be able to use the Evaluation Note Tempates that you've created, the Profile and Case Type should match to the patient's case.
Scope:
This procedure covers:
- Creating new templates via direct setup or saving from existing notes
- Accessing the evaluation notes section
- Template compatibility requirements (profile/case type matching)
Procedure:
Method 1: Create a New Template Directly
- Go to Settings > Evaluation Note Templates
- Click Create New Template (top-right corner)
- Configure template content and settings
- Save the template
Method 2: Save an Existing Note as a Template
- Navigate to:
- Options Tab > Patients > Select a patient
- Notes Tab > Actions > View Notes List
- Click New Notes > Add Evaluation Note
- Draft the note, then select Save as Template
- Save as a draft template
Important Note:
- Templates must match the profile and case type of the cases they’re applied to.

The evaluation note template portion of the settings contains all templates therapists can use when writing evaluation notes. New templates can be created by clicking the "Create New Template" button in the top right or going through the notes and clicking "Save as Template" where you can save the note as a draft.
From the Options Tab > Click on Patients > Select a Patient
Once a patient is selected click on the Notes Tab > Click on Actions on the Selected Case then select View Notes List
Under Actions Click on New Notes > select Add Evaluation Note
Please note that when using a note template, the profile and case type of the template created must be identical to the profile and case type of the case being worked on.
