How can I create my own Consent/Intake forms in Hellonote?

We are glad to announce that users can now create their own Consent/Intake forms in Hellonote. 

To get this feature activated users may reach out to the support group 

To do this simply navigate to Settings > Consent Intake 

Click on Create new Dynamic Form 

Select the type of form you would like to create 

Drag the content component as shown below and it will open an Editor window 

This button allows users to input he Logo and align the logo as well 


Here is an example of the content and the edit button to make changes if necessary 

The editor has multiple components

1. Font Family/Size

2. Bold/Italics/Link 

3. Bullets/Numbering 

4. Uploading images, Black quotes, Graph  

5. Text alignment

6. Undo/Redo

For advance customization users may also take advantage of API function, Conditional, Logic and Layout 

CSS Function is also an available tool to use  


Signature is also added by dragging the signature component below 

Make sure to name the file and click Save 

1. Once saved the form will be available as an active form 
2. All Active forms are displayed here by default

3. To select inactive forms to be use again just click on the in active button 

Creating an Intake form gives out more options 

Select Intake from the Form Type Selection 

Multiple elements can be dragged for multiple functions 

Make sure to click on Save upon finalizing the form 

To activate and send this newly created forms ensure that Enable Dynamic Forms is clicked.
