How do I send consent/intake forms to my patients ?
Purpose:
To guide clinicians through the process of electronically distributing and collecting digital consent and intake forms from patients via HelloNote EMR, streamlining the onboarding process while maintaining compliance.
Scope:

- Selecting and sending forms to individual patients
- Available form types (consent and intake)
- Form delivery and return process
- Accessing completed forms in patient documents

Procedure:
- Locate the Patient:
- Select the patient from your Patient List
- Initiate Form Send:
- Click the pencil icon (Send Consent/Intake Forms)
- Select Forms:
- Choose from dropdown menus:
- Consent Forms
- Intake Forms
- Select multiple forms if needed
- Choose from dropdown menus:
- Deliver Forms:
- Click Save to automatically email forms to patient
- Process Completed Forms:
- Signed forms auto-upload to Patient Documents tab
- Options for:
- Viewing
- Printing
- Faxing
- Sharing via portal
