How to generate a copy of a claim form in Inovalon?

Once a claim has been transferred from HelloNote to Inovalon, you can generate a copy of the CMS-1500 claim form directly from the claim in Inovalon.

The CMS-1500 form provides a formatted copy of the claim that can be reviewed or downloaded for recordkeeping and reference.

Steps to Generate the CMS-1500 Claim Form

Step 1: Open the Claim in Inovalon

1. Log in to Inovalon.

2. Locate the claim that was transferred from HelloNote.

3. Open the claim to view its details.

Goal: Access the correct claim before generating the form.

Step 2: Click Print

1. From the open claim, locate and click Print.

2. Review the available print options.

   

Step 3: Select CMS-1500

1. Select CMS-1500 from the available options.

2. Allow the system to generate the claim form.

The system will generate a copy of the CMS-1500 claim form for you to review or download.
