How do I create a Placeholder Evaluation Note?

Purpose:

To guide users through the process of creating a placeholder evaluation note in HelloNote EMR, enabling the seamless transfer of patient treatment documentation from another EMR system while maintaining continuity of care records.

Scope:

This procedure covers:

• Accessing and creating a new evaluation note template in Settings
• Configuring a placeholder template for migrated patient cases
• Entering essential placeholder information while deferring complete documentation
• Saving the template for future use with transferred patients

Procedure:

1. Navigate to Settings > Evaluation Note Template
2. Click Create New Template
3. Name the template (e.g., "Placeholder Profile")
4. Select applicable case types
5. Under Action > View/Edit:
   1. Skip date fields initially
   2. Optional: Select place of service
   3. In Reason for Referral: Enter "See original evaluation"
   4. Repeat "See original evaluation" in all sections (Subjective, Objective, etc.)
6. In Treatment Plan:
   1. Add recommended treatment codes (e.g., PT evaluation codes)
   2. Set prognosis (e.g., "Good")
   3. Leave certification date/duration blank
7. In Plan/Assessment: Enter "See original evaluation"
8. Save the template
