How to create an Evaluation Note?

Purpose:

To guide clinicians through the process of creating an evaluation note

PLEASE NOTE: A case needs to be create first before you can create an evaluation note

Procedure:

1. Select your patient on from your Patient List
2. Go to the Notes Tab
3. Open the case by clicking on the Case Name or by clicking Actions > View Note List
4. Click Add New Note
5. Select Evaluation Note
