How do patients request appointments on the patient portal?

Log into the patient portal by going to hellonote.com then click log in
Click on appointments
Scroll at the bottom part Request for Booking
Select the Provider, the visit type and the date and time of your desired schedule
Once the date and time is selected, a pop up window will appear, you can add comments then click on submit
Once submitted, a confirmation pop up will show
