How do patients sign consent and intake forms on the patient portal?

Patients can sign their consent and intake forms once they log in the Patient Portal

1. Go to hellonote.com then click log in
2. Once log in, click on either consent or intake form and select the form you need to sign from the drop down
3. Once a form is selected, it will provide you the contents of the form and a place for the patient to sign. Sign and select the date then submit
