How do patients upload documents on the patient portal?

Patients can upload their own documents by logging into the Patient Portal

1. Go to hellonote.com and click log in
2. Click on Documents then click New
3. Fill out the details as shown. If the document the patient needs to upload will be faxed out, make sure to add .pdf at the end of the file name like the example shown below then click save when done.
