How to add the KX modifier?

Purpose:
To properly apply the KX modifier to claims and patient cases to indicate that Medicare therapy cap requirements have been met and to ensure correct billing going forward.

Scope:
This procedure covers adding the KX modifier to individual claims in the billing module and enabling it permanently for future notes in the patient’s case settings.

Procedure:

Part 1: Adding KX to an existing claim

1. When you see CO 119 (claim adjustment reason code), note the patient’s number.
2. In Professional Claims, enter that number in the Patient Control Number field and click Search.
3. Locate the correct Date of Service.
4. In the Modifiers column for each relevant HCPCS code (e.g., 97112, 97530, 97110), add KX after the existing modifiers (e.g., GP, CO, KX).
5. Save, Validate, and send the claim.

Part 2: Enabling KX for all future notes

1. In HelloNote, go to the patient’s Notes.
2. Click the Case Settings tab (gear icon).
3. In the Apply KX section, check the Apply KX box.
4. Click Save.
