How to resolve patient demographics not carrying over in Claims?
Purpose:
To provide a solution for ensuring accurate patient demographic transfer to claims when policy holder information is incorrectly configured.
Scope:
This article covers:
- Policy holder information verification
- Claim regeneration process
Excludes: - Initial claim submission
- Insurance-specific demographic requirements
- Bulk claim updates
Procedure:
- Verify Policy Holder Settings:
- Ensure "Other Policy Holder" checkbox is unchecked
- Confirm Policy Holder Information section is correctly populated
- Regenerate Claims:
- Delete affected claims in Inovalon
- Navigate to Billing > Submitted in HelloNote
- Re-upload corrected claims

1. Make sure that the other Policy Holder is Uncheck
This then references the section right below where it says "POLICY HOLDER INFORMATION"
2. Once updated you can delete the claims in Inovalon, then go to Billing > Submitted in HelloNote and re-upload the claims, and it will show properly
